Abstract

|
The antibody response to a 23-valent pneumococcal polysaccharide vaccine was studied in 21 healthy, 48 nephrotic and 29 splenectomized children. Antibody levels before and 4 weeks after immunization against the six pneumococcal serotypes(6B, 9V,
14,
18C, 19R and 23F) were determined by an enzyme-linked immunosorbent assay. The healthy children responded well to vaccination with a mean fold increase of 2.6 in postvaccination antibody titers compared to prevaccination titers. In children with
nephrotic syndrome and splenectomized children an antibody response did not differ significantly from of healthy children. Howere, they had significantly lower geometric mean antibody titers for some antigens both before and after immunization.
Adverse
reactions were always mild and transient, consisting of local pain and/or erythema or swelling in 41% and fever above 38.5% in 3% of the chidren These results indicate that pneumococcal polysaccharide vaccine in immunogenic and safe in highrisk
as
well
as healthy children.
